Consent to Telehealth

1. Acceptance of Consent
By clicking "I Agree," checking a related box, or using any acceptance method provided through the Service, you acknowledge and accept this Telehealth Consent ("Consent"). If you do not agree, do not create an account or use the Service. Acceptance on your behalf by another party constitutes agency authority.
2. Emergency Situations
If you are experiencing a life-threatening emergency, call 911 or the 988 Suicide & Crisis Lifeline. The Service is not appropriate for urgent or emergency medical situations.
3. Purpose
This Consent provides information about telehealth and authorizes physicians, nurse practitioners, physician assistants, and other licensed professionals ("Providers") to deliver care through HeyFeels' digital platform ("Service").
4. What is Telehealth?
Telehealth is the delivery of healthcare services remotely through electronic communication. It may involve:
- Remote diagnosis, treatment, and follow-up
- Sharing medical records, images, or personal health data
- Real-time communication (video, audio, messaging)
- Device-generated health data
You may choose in-person alternatives at any time. Discuss options with your Provider.
5. Benefits of Telehealth
- Convenient and efficient access to care
- Flexible scheduling
- No in-office visits required
6. Risks of Telehealth
Potential risks include:
- Technology limitations or malfunctions
- Inaccurate or delayed diagnosis
- Limited ability for physical exams or tests
- Delayed responses from Providers
- Security breaches or data loss
- Potential exposure of private data (e.g., via email)
- Treatment limitations due to licensing, jurisdiction, or incomplete records
7. Emergency & Follow-Up
For emergencies, call 911. Telehealth is not suitable for all situations. Providers may not respond immediately. Use the secure message feature for non-urgent communications.
8. Data Privacy
The Service uses encryption and other safeguards to protect your information. Your data will not be shared without your consent, except:
- As required by law
- For treatment, billing, or healthcare operations
- In cases of mandatory reporting (e.g., abuse or threats)
- As described in your Provider’s Notice of Privacy Practices
HeyFeels cannot guarantee the security of your email or personal communication methods.
9. Your Acknowledgments
By accepting this Consent, you acknowledge and agree to the following:
- Care is provided remotely by licensed Providers, which may include non-physicians (e.g., NPs, PAs, counselors)
- Telehealth technology may be in beta/testing and may contain bugs or defects
- Diagnostic and lab services provided through the platform may contain limitations
- No specific results or outcomes are guaranteed
- Your condition may not improve and could worsen
- Not all conditions are treatable via telehealth
- Your Providers will determine if telehealth is appropriate
- No sessions will be recorded by Providers or users
- You may withdraw consent at any time by written notice, though Providers on the platform do not offer in-person care
You also understand:
- It is your duty to provide complete, accurate medical and personal information
- Providers may determine a need for in-person or specialist care outside of the Service
- Your prescription may be filled by any pharmacy, including HeyFeels’ partners, and transferred between them
- You must pay the full cost for all services and prescriptions and will not submit claims to Medicare, Medicaid, or private insurers
10. Complaints and Contact
If you have concerns about a medical professional, contact the appropriate Medical Board in your state. For other questions, please contact us via the platform support system.
By continuing to use the Service, you confirm your understanding and acceptance of this Telehealth Consent.
